For Referral Partners

SSAs, case managers, therapists, and healthcare providers trust us with the people they serve, and we take that seriously. We respond to referrals promptly, keep you informed, and work alongside you to support each person’s plan.

What we provide

For individuals with developmental disabilities: DODD-certified Homemaker/Personal Care (HPC), Participant-Directed HPC (PD-HPC), HPC Transportation, and Residential Respite, billable under the Individual Options (IO), Level One, and SELF waivers. We serve children and adults.

For older adults: Personal care, companion care, memory and dementia care, respite, 24-hour and overnight care, mobility assistance, transportation, medical appointment support, and care coordination. We accept long-term care insurance and private pay.

Service area: Hamilton, Butler, and Warren counties.

How referrals work

Step 1. Send us the referral by phone or through the form below.
Step 2. We reach out to the individual or family to learn about their needs and goals.
Step 3. We recommend caregivers, and the family interviews them and makes the final choice.
Step 4. We coordinate with you on the service plan and keep you updated as care begins.

A resource to share with families

Many families you serve may be able to be paid for the care they already provide under Ohio’s DODD waivers. Our Family Caregiver Guide explains how it works. Feel free to download it and share it.
